Santiago de Cali — known simply as Cali — is Colombia’s third-largest city, with 2.2 million people in the heart of the Valle del Cauca. It sits at 1,018 meters (3,340 feet) in a wide valley flanked by the western and central ranges of the Andes, giving it a warm, tropical climate of 75-85°F (24-30°C) year-round.
Colombians call it La Sucursal del Cielo — the Branch of Heaven. And La Capital de la Felicidad — the Capital of Happiness. These are not just nicknames. Caleños (as the residents are called) are famously warm, joyful, and proud of their city. The energy here is different from Medellín’s cosmopolitan polish or Bogotá’s intellectual depth — Cali runs on music, dance, and an infectious love of life that visitors feel immediately.
Most medical tourists do not know about Cali. That is exactly why it deserves your attention.
Because Cali is home to Fundación Valle del Lili — the #1 ranked hospital in all of Colombia according to Newsweek’s World’s Best Hospitals 2025, with a score of 92.97%. And it is home to Centro Médico Imbanaco, ranked #6 nationally, JCI-accredited since 2009, and one of the longest-established international patient programs in the country.
Cali offers world-class medicine without the medical tourism crowds. Lower prices than Medellín. Less congestion than Bogotá. And a cultural experience — salsa, Afro-Colombian cuisine, Pacific coast proximity — that neither of those cities can replicate.
The Hospitals
Cali punches far above its weight medically. The city has two of Colombia’s top 6 hospitals:
| Hospital | Colombia Rank | Score | Specialties |
|---|---|---|---|
| Fundación Valle del Lili | #1 | 92.97% | Comprehensive — transplants, oncology, cardiac, neurosurgery, trauma, pediatrics. The most highly ranked hospital in Colombia. |
| Centro Médico Imbanaco | #6 | 86.43% | JCI-accredited since 2009. Established international patient program. Cardiac, orthopedics, fertility, oncology. |
| Clínica de Occidente | #16 | 75.41% | General surgery, internal medicine |
Fundación Valle del Lili is not just Colombia’s top hospital — it is ranked among the best in the world by Newsweek. It is a 550+ bed tertiary care center with over 1,200 physicians, a transplant program that performs more organ transplants than any other hospital in Colombia, and a research department that publishes in top international medical journals.
For patients seeking the absolute highest-ranked medical facility in Colombia, the answer is not Medellín or Bogotá. It is Cali.

What Cali Does Best
Complex and specialized procedures
Valle del Lili’s breadth of subspecialties makes Cali an excellent choice for:
- Organ transplants: The highest volume transplant program in Colombia (kidney, liver, heart, lung, bone marrow)
- Cardiac surgery: Advanced cardiac surgical and interventional programs
- Oncology: Comprehensive cancer center with surgical oncology, chemotherapy, radiation, and immunotherapy
- Neurosurgery: Complex spinal and brain procedures
- Pediatric surgery: One of the top pediatric surgical programs in Latin America
Cosmetic surgery
Cali has a long and deep tradition in cosmetic surgery — in fact, many Colombians consider Cali, not Medellín, to be the original capital of Colombian plastic surgery. The city has a high concentration of board-certified plastic surgeons and a culture that embraces aesthetic enhancement openly. Cosmetic surgery prices in Cali tend to be 10-20% lower than Medellín for comparable quality. See our Cosmetic Surgery guide.
Fertility and IVF
Centro Médico Imbanaco operates one of Colombia’s most established fertility programs, with a modern embryology laboratory and a team of reproductive endocrinologists experienced in treating international patients. See our IVF guide.
Orthopedic surgery
Both Valle del Lili and Imbanaco have strong orthopedic departments performing joint replacement, spine surgery, and sports medicine procedures. See our Orthopedic Surgery guide.
Where to Stay
Ciudad Jardín
Ciudad Jardín (Garden City) is Cali’s most exclusive residential neighborhood. Tree-lined streets, large homes, private gardens, and a quiet, upscale atmosphere. It is close to Valle del Lili and several private clinics.
- Luxury serviced apartments and vacation rentals available ($800-$2,500/month)
- Quieter and more residential than the tourist-oriented neighborhoods of Medellín or Cartagena
- Excellent for patients who want privacy and tranquility during recovery
Granada and El Peñón
Granada is Cali’s culinary and nightlife epicenter — a tree-lined neighborhood packed with restaurants, bars, boutique shops, and cafes. El Peñón, adjacent to Granada, has a bohemian character with art galleries, independent cinemas, and a creative scene.
- Walking distance to dozens of excellent restaurants
- Boutique hotels and Airbnb apartments available ($40-$120/night)
- More vibrant and social than Ciudad Jardín

Hotels
- InterContinental Cali: The city’s premier international hotel. Modern, well-appointed, pool, excellent service. Located in the upscale south of the city near the medical corridor.
- Hotel & Centro de Convenciones Dann Carlton Cali: Full-service hotel with good access to hospitals.
- Movich Hotel de Cali: Contemporary design hotel in a central location.
- Boutique options in Granada and San Antonio: Several smaller, character-driven hotels have opened in Cali’s trendiest neighborhoods.
Hotel rates in Cali are generally 20-30% lower than equivalent properties in Medellín or Bogotá, reflecting the city’s lower cost of living.
Where to Eat
Cali’s food scene is distinct from the rest of Colombia. The city’s Pacific coast influence brings Afro-Colombian flavors — coconut, plantain, tropical seafood, and aromatic spices — that you will not find in Medellín or Bogotá.
Fine dining and upscale
- Platillos Voladores: Contemporary Colombian cuisine with Pacific coast influence. One of Cali’s most celebrated restaurants. Creative tasting menus in an intimate setting.
- Ringlete: Farm-to-table concept using ingredients from the Valle del Cauca. Modern technique, deeply local flavors.
- Zahaví: Mediterranean-Colombian fusion in Granada. Sophisticated, with an excellent cocktail program.
- La Comitiva: Upscale Caleño cuisine. Traditional dishes reimagined with premium ingredients.
Traditional Caleño food
- Lulo Bar Restaurante: Beloved local institution serving classic Caleño dishes — sancocho de gallina (hen soup), arroz atollado (rice stew), aborrajados (fried plantain with cheese).
- El Zaguán de San Antonio: Set in the charming San Antonio neighborhood on a hilltop. Traditional food, beautiful views of the city.
- Cholados: Cali’s signature street food — a mountain of shaved ice, fresh tropical fruit, condensed milk, and flavored syrups. Best at the traditional cholado stands near the RÃo Cali.
Pacific coast cuisine
- Restaurante Mar y Luna: Authentic Pacific coast seafood. Ceviches, encocados (coconut-based fish stews), and fried fish with patacones. The flavors of Buenaventura brought to Cali.
- La Tartine: French-Colombian in Granada. Excellent for a lighter, refined meal.
What to Do During Recovery
Gentle activities
- San Antonio neighborhood: A hilltop colonial neighborhood with a beautiful park, a small chapel (Capilla de San Antonio), panoramic views, and an artisan market on weekends. Shaded benches, street vendors, and one of the most peaceful spots in the city.
- El Gato del Río: A riverside walkway lined with whimsical cat sculptures by Colombian artist Hernando Tejada. A relaxed walking path along the Río Cali with cafes and shade.
- Museo La Tertulia: Cali’s modern art museum. Contemporary Colombian and Latin American art in a striking building on the riverbank. Free on certain days.
- Shopping: Chipichape and Unicentro are Cali’s largest malls, with international brands and excellent food courts.
- Spa and wellness: Several high-end spas in the south of the city and in the surrounding countryside offer day treatments.
More active
- Salsa dancing: Cali is the salsa capital of the world. Even if you have never danced, you owe yourself a salsa lesson in Cali. Schools like Swing Latino offer group and private classes for beginners. For watching, TinTín Deo and La Topa Tolondra are legendary salsa clubs where the dancing is as athletic as it is beautiful. For patients further along in recovery only.
- Cristo Rey: A 26-meter Christ statue (similar to Rio de Janeiro’s, but less crowded) on a hill overlooking the city. Accessible by car or taxi. Panoramic views of the valley.
- Hacienda El Paraíso: A beautifully preserved 19th-century sugarcane hacienda 45 minutes from Cali. The setting of María, Colombia’s most famous romantic novel by Jorge Isaacs. Set among rolling green sugarcane fields with Andean mountains in the background.
- Lago Calima: A reservoir lake about 2 hours from Cali, popular for windsurfing, kitesurfing, and sailing. Beautiful setting in the mountains. Day trip or overnight.
- Pacific coast excursion: Buenaventura and the Pacific coast beaches (Juanchoí, Ladrilleros) are 3-4 hours from Cali. Pristine beaches, whale watching (July-October), and Afro-Colombian culture. A unique experience you cannot replicate from Medellín or Bogotá.
Getting Around
- Uber: Available and reliable. $2-$5 for most rides within the city.
- MIO (Masivo Integrado de Occidente): Cali’s bus rapid transit system. Functional but not recommended for post-surgical patients.
- Private driver: $40-$70/day. Useful for day trips and hospital transportation.
- Walking: Granada, San Antonio, and El Peñón are walkable. The city is generally flat in the central areas.
Getting There
Alfonso Bonilla Aragón International Airport (CLO), located in Palmira, approximately 30 minutes from the city center.
Direct flights:
- Miami (MIA) → Cali (CLO): 3.5 hours — Avianca, American Airlines
- Fort Lauderdale (FLL) → CLO: 3.5 hours — Spirit, JetBlue
- New York (JFK) → CLO: 5.5 hours — JetBlue (seasonal), connections via Bogotá
- Bogotá (BOG) → CLO: 1 hour — multiple daily flights (Avianca, LATAM)
- Medellín (MDE) → CLO: 1 hour — Avianca
For cities without direct service to Cali, connecting through Bogotá adds only 1 hour of flight time.

The Climate
- Temperature: 75-85°F (24-30°C) year-round. Warmer than Medellín, cooler than Cartagena. Cali’s altitude (1,018 meters) keeps it from being oppressively hot.
- Humidity: Moderate. More comfortable than Cartagena’s Caribbean humidity.
- Rain: Two drier periods (January-March, July-August) and two wetter periods. Rain typically comes in afternoon showers.
For recovery: Cali’s climate is warm but not extreme. It is comfortable for walking and outdoor activity without the swelling concerns of Cartagena’s heat or the need for layering in Bogotá’s cooler weather. For many patients, it hits the sweet spot between Medellín’s spring and Cartagena’s tropics.
Why Choose Cali?
Cali is the right choice for patients who:
- Want the highest-ranked hospital in Colombia — Valle del Lili is #1 nationally. If your procedure is complex and you want the best-ranked facility, this is it.
- Want lower costs — Cali’s cost of living is lower than Medellín and significantly lower than Bogotá. Medical procedures, hotels, food, and daily expenses are all more affordable.
- Want a less touristy experience — Cali has not been “discovered” by the medical tourism industry the way Medellín has. Fewer package tourists, more authentic Colombian experience.
- Love music and dance — Salsa is not a performance in Cali, it is life. If cultural immersion matters to you, there is nothing like it.
- Are interested in Pacific coast culture — Cali’s proximity to the Pacific coast gives access to Afro-Colombian culture, cuisine, and beaches that are inaccessible from other Colombian cities.
- Need cosmetic surgery at the best value — Cali’s tradition in plastic surgery is deep, and prices are typically lower than Medellín.
Cali may be less ideal for patients who:
- Want the largest international patient infrastructure (Medellín has more recovery houses, English-speaking services, and medical tourism-specific support)
- Prefer cooler weather (Cali is warmer than Medellín)
- Want a beach vacation as part of recovery (Cartagena is the better choice for that)
